New York State law (Chapter 281) permits schools to request an oral health assessment at the same time a health examination is required (preK or K, 1, 3, 5, 7, 9 & 11). Your child may have a dental check-up during this school year to assess his/her fitness to attend school. 

The dental certificate must contain a report of a comprehensive dental examination performed on your child.  The form is below or you can obtain one from the Health Office.  We also have a list of dentists that accept Medicaid and provide services on a sliding fee scale for those that may need financial assistance.
